Transcript:
You must submit one transcript from each post-secondary institution attended, regardless of whether or not a degree was earned or the credits are reflected on a separate transcript as transfer credit. If documents are not in English, also provide a certified English translation of each document. Translations certified by the degree granting university are considered official. Third party translations are not considered official. Failure to provide all academic documents will result in delays in processing your application materials. Please upload copies of academic transcripts (and diploma/degree certification, if applicable) with your application. If you have previously attended or are currently attending Ohio University, you must indicate this on your applciation. Your OHIO transcripts will be obtained by the Graduate College on your behalf.
Curriculum Vitae:

Personal Statement:
Personal Statement - 500 words maximum. (Do not mail if you uploaded it to your online application.)
Recommendation Letter:
Three letters of recommendation. You should opt to have these submitted online. Once your application is submitted and payment has cleared, the system will contact the persons you have listed and give them instructions for uploading their letters.
Financial Support Form:
Submiting a Financial Support Form with the application is not required, but it is recommended. The United States Citizenship and Immigration Services require that Ohio University establish that students have sufficient funds for the first year of study before a Form I-20 or DS-2019 Certificate of Eligibility can be issued. Having a complete Financial Support Form on file at the point you are admitted will speed up processing of your Certificate of Eligibility.
