You must have received a bachelor's degree from an accredited institution. Additionally, you must submit official copies of your transcripts from every insitution you have attended with your application.Applicants should plan on having all transcripts from any non-US colleges or universities they have attended evaluated by World Education Services.

Personal Statement:
Your personal statement should be 2 pages in length, single spaced, and should address your career goals and purpose for applying to the program. Please make sure to include any past work/internship/volunteer experience and reasoning for how this program will serve your future goals
Recommendation Letter:
While not required, it is highly encouraged that at least one letter of recommendation come from a faculty member who can attest to your academic performance.
Financial Statement :
The financial statement is only for admitted applicants and is submitted to the Office of International Admissions when requested.It provides the Colorado School of Public Health with information related to your ability to pay for the certificate program. Please note, the financial statement must be translated into English.

The I-20 is only for admitted applicants and is submitted to the Office of International Admissions when requested.The I-20 form gives the Colorado School of Public Health the information needed to request your student visa from the United States government.
