Transcript:
You must have a bachelor's and master's degree, or have completed substantial coursework toward such a degree.Additionally, you must submit official copies of your transcripts from every insitution you have attended with your application. Applicants should plan on having all transcripts from any non-US colleges or universities they have attended evaluated by World Education Services.
Curriculum Vitae:
Please submit a recent copy of your resume or curriculum vitae (CV).
Personal Statement:
Your personal statement should be two pages in length, single spaced. It should describe your past experience(s) as they relate to your interest in public health, career goals, skills that you will bring to the program, purpose for submitting your application, and how the program will benefit you.Please also identify the faculty member with whom your research interests align.
Recommendation Letter:
Four (4) Letters of Recommendation.While not required, it is recommended that at least one letter come from a faculty member who can attest to your academic performance.
Financial Statement :
The financial statement is only for admitted applicants and is submitted to the Office of International Admissions when requested.It provides the Colorado School of Public Health with information related to your ability to pay for the certificate program. Please note, the financial statement must be translated into English.
I-20 Form:
The I-20 is only for admitted applicants and is submitted to the Office of International Admissions when requested.The I-20 form gives the Colorado School of Public Health the information needed to request your student visa from the United States government.
